There was no problem whatsoever at 3:40, he made awkward expressions throughout the performance, this probably was just another one...
He plays an arpeggio with both hands even if the score shows only right hand the second time he plays the main theme, then at 2:02 he plays plain octaves where Mozart wrote broken octaves, guess is just his own addition to Mozart's score... nothing wrong to that, I loved the whole thing....

In spite of his clowning and his obvious desire to get on the conductor's nerves Horowitz kicks major piano butt. Like Rubinstein this old school musician is cool. No facial ticks, no posturing. He comes to play. A lesson for this generation of emotive pianists.

It's the video for a studio audio recording. They probably redid many takes. Thats also why he has the sheet music in front of him- he wouldn't use it normally.
